The beauty and harmony of the face are highly related to the appearance of the teeth and their relations with each other and with the lips and other parts of the face. But good appearance is not the only important goal of orthodontic treatment, where the appropriate functions of the teeth, tongue, muscles, and bones are considered as the main interest along with the good appearance.

NORMAL TEETH AND JAWS RELATIONS

For good appearance and best functional performance of the mouth components, the teeth in each jaw must be arranged in a certain arch form (dental arch) that is correlated in size and shape to the other components of the facial bones, both upper and lower dental arches must be related to each other in a harmonious relationship that ensures the best functions of speech and chewing.

Moreover, good teeth relations in each dental arch and good dental arches relations will serve to reduce the possibility of dental cavities (dental caries) and gum diseases through the facilitation of what is so-called self-cleansing mechanisms, and also give the chance of normal movements of the temporomandibular joints (TMJs) (the two joints between the lower jaw and the skull at both sides) which will prevent their dysfunction and pain.
The psychological impact of appearance is another very important issue, where appearance does affect the social life of most people. 

 ORTHODONTIC PROBLEMS

When any of the teeth relations, dental arches or jawbones are not normally related, an orthodontic problem arises, the severity of the problem is related to the severity of its effects on the functions and appearance, of course, it is in many times more subjective than objective, because the degree of the impact on a person (physically, psychologically or socially) differs amongst people according to the perception of the problem by that person.

Orthodontic problems can arise from genetic or acquired origins. Genetically originated orthodontic problems arise due to the effect of the genes in the formation of teeth and bones; they are inherited and transmitted to the siblings from their parents. Whereas acquired problems arise because of external factors that affect teeth and bones after birth, such factors for example include; early loss of milk teeth, traumas, and bad habits like thumb sucking or pushing the tongue between the front teeth.

 PREVENTION AND TREATMENT

The treatment of some congenital orthodontic problems, like cleft lip or palate, usually begins at an early age of life using simple appliances in conjunction with surgical interference. On the other hand, factors suspected to cause an acquired orthodontic problem must be stopped as soon as they discovered to prevent them from developing the problem.

Prevention of orthodontic problems depends on preserving milk teeth until the normal time of their replacement by the permanent teeth, in addition to the use of some appliances that prevent the effect of some “pressure habits” from altering the normal growth of the bones and teeth.

 

WHEN TO BEGIN ORTHODONTIC TREATMENT?

Orthodontic problems can be divided into two categories: the first is problems of bone growth, and the second is problems of teeth alignment. Treatment of bone growth problems must begin during the time of the growth period to give a good chance to affect and/or redirect the speed, amount and direction of bone growth, i.e. to increase retarded bone growth or to reduce increased bone growth. Whereas the treatment of teeth discrepancies usually starts after all milk teeth are replaced by their permanent successors, unless the discrepancy interferes with normal growth and development of teeth or bones, in these circumstances an early treatment is important bearing in mind that this stage of treatment will not necessarily delete the need for later comprehensive treatment.

ORTHODONTIC TREATMENT WITH APPLIANCES

Orthodontic treatment usually depends on the use of appliances, these appliances fall into two main types; removable and the fixed orthodontic appliances. Removable appliances (can be removed by the patient) are used in some initial stages of the comprehensive treatment they are effective in certain simple discrepancies but they fail in more difficult cases. Fixed orthodontic appliances (fixed on the surfaces of the teeth and cannot be removed by the patient) are used to treat most of the cases; there are several types of them depending on the philosophy of the treatment plan (e.g. Edgewise, Roth, and the self ligature systems). Some of the fixed appliances are made of metal and others are made of teeth-colored or clear material. Some of them are fixed on the outer surfaces of the teeth and others are fixed on the inner surfaces.

INVISIBLE ORTHODONTIC APPLIANCE

This appliance is considered the most recently developed method in orthodontic treatment. The method depends on the use of a series of appliances that are designed by the use of a computer program; each one of them is used for a certain time to correct an increment of the problem, with the progression of time and with the use of the successive appliances the case is being treated gradually until the use of the last one.

SURGICAL ORTHODONTICS

In certain cases where the problem of bone growth couldn’t be treated early, and when the bone growth discrepancy is well developed, the conventional orthodontic appliances and methods fail to correct the problem. In such circumstances the need for some kind of surgical interference is inevitable, in these cases, some orthodontic treatment, in the beginning, will be necessary to prepare the teeth for the surgical part, then after the surgery is performed, another period of orthodontic treatment is needed for the final leveling and alignment of the teeth in their final desired positions.
